Chantix - Drug Interactions

There have been no clinical pharmacokinetic drug interactions in regard to the use of Chantix (varenicline) with warfarin, transdermal nicotine, metformin and cimetidine.

The inhibition of cytochrome P450 enzymes has not been reported in vitro studies. The inhibition of human renal transport proteins has not been seen at therapeutic concentrations.

It has been reported that a dose adjustment of Chantix will not be required in the case of active renal secretion which is mediated by human organic cation transporter which is OCT2 as result of increase in systemic exposure has not been clinically useful.

• In the case of Cimetidine – this is an OCT2 inhibitor, the study shows it increased the systemic exposure of varenicline by 29% which was a result of reduction in varenicline renal clearance.
  
  
• Metformin – The steady state of pharmacokinetics of metformin was not altered when it was co – administered in around 30 smokers. 
  
  
• Digoxin – Chantix when used along Digoxin did not show any interaction.
  
  
• Warfarin – There was no reaction which was seen in the case of varenicline along with warfarin in around 24 smokers.
  
  
• Bupropion - The steady state pharmacokinetics of this drug was not altered when seen in around 46 smokers, though the safety of both varenicline and Bupropion has not been stated.

Chantix with Other Therapies

• Nicotine replacement Therapy - Reactions such as headache, nausea, vomiting, dizziness, dyspepsia and fatigue were reported to be higher when Chantix was administered along with Nicotine replacements therapy. It has been noticed the usage was also discontinued by some where adverse reactions were visible.
